Singapore achieves very good health outcomes while maintaining access and affordability. But our healthcare system, which is anchored by strong hospitals and specialists, must transform itself in order to serve Singaporeans well in a fast-changing future.The epidemic of chronic diseases like diabetes means prevention has to start early and continue long term. Patients with chronic diseases require lifelong support so they can manage their conditions well and avoid serious complications. Meeting the needs of the rising number of older people, who may have multiple health and social problems, demands fresh approaches.Our health system therefore needs new wellness and disease prevention strategies that help citizens stay well and age well in the community. Strong primary health and community-based services are central to providing the holistic, long-term care and support necessary for patients with chronic diseases especially those who are elderly. These changes have to be effected while keeping costs affordable and health manpower demand sustainable.The MOH Office for Healthcare Transformation (MOHT) was formed in 2018 to help accelerate, drive and contribute to the transformation of our healthcare system. MOHT focuses on a number of more fundamental and longer-term areas. It works with partners to design and pilot new care models, and concurrently collaborates with MOH, IHIS and other stakeholders to develop critical enablers of scaling such as technology and IT, data analytics, and finance and incentive redesign. MOHT endeavours to draw all these together as effective new end-to-end system-level solutions that have a transformative impact on health outcomes and care.
