Our Admission nurses use their clinical expertise to evaluate each patient's unique needs and provide immediate care, support and education for the patient and family.- Our admission and social work staff explore all potential resources, including community services and placement (if needed), ensuring that patients and families receive individualized care to meet their long-term care goals.- OneCare Hospice is committed to community education and outreach.
