The first primary health care medical center was established on January 4, 1999 in the vicinity of Dhok Choudhrian, Rawalpindi. A philanthropist provided a newly constructed building free for this purpose. The Medical center functioned at this location for three years, and fully established its credibility amongst the poor people. During this period 148,590 patients were treated mostly free of cost; of these 80% were women and children. After which it moved to its own building in Chaklala Scheme III. The construction of this building commenced on 31st March 2001 and was completed by October 2002.In 2004, AL Mustafa Trust decided to develop medical facilities in rural areas depending on availability of financial resources.
