We operate a patient-centered outpatient multi-specialty clinic that treats our patient's health needs by combining physical health, behavioral health, and care coordination to provide value-based care. Our model limits the locations to a certain amount of patients so we can provide a high staff to patient ratio that provides unparalleled care in a safe, comfortable home-like environment.Our programs include primary care, partial hospitalization, intensive outpatient, and non-traditional cognitive therapies and care coordination all united to help our patients live productive and joyful lives.
