Baltimore CONNECT is a 501(c)(3) organization comprised of community-based organizations and the Johns Hopkins Health System to better link social and healthcare services across Baltimore. Our mission is to create and strengthen this collaborative network of community, faith-based and neighborhood organizations that advocate for the needs of people in Baltimore, and CONNECT them to comprehensive and coordinated health and human services. We envision a future in which health care entities, social service organizations and people in the community work together seamlessly to achieve optimum health and well-being for all.
