Care Collaborator was developed to combat the problem created in the aged care sector with the introduction of the new aged care reforms. From 1 July 2015, a home care package consumer has to receive a Home Care Agreement including a Care Plan and Individualised Budget. At the moment Home Care Providers are using low-tech manual spreadsheets or Access databases and having to go back to the office to transfer the data into the documents, then return to the consumer's home to have them signed. If further changes are required, the process has to be repeated making it a very inefficient system. Care Collaborator uses cloud technology and has been designed to be used on a mobile device making it easy for the care manager to collect the data while assessing the consumer's needs. The data collected populates the documents which can be reviewed with the consumer and signed immediately using this technology. The care manager can then email the documents to the consumer, completing the process and arrange the services identified in the care plan immediately. This reduces the waiting time for the consumer, making it a very efficient process which is essential as this group of people are the most vulnerable and need to feel secure in their homes. This solution will save the Home Care Provider time and money and allow them to focus on the client's needs rather than an onerous manual process.
