CCS Healthcare has developed a Telehealth and Chronic Care Management program specifically for Assisted Living Facilities. We offer on demand access to Telehealth Providers 24 hours a day 7 days per week! We have developed a system that gives patients better access to care by leveraging technology. This results in patients getting the care that they deserve when they need it. The program works through 24/7/365 Telehealth coverage via a telehealth cart installed in your facility. The providers in the CCS Healthcare group are all board certified in Emergency Medicine. Our goal is to decrease unnecessary Emergency Department utilization, reduce readmissions, and perform more care in place. We are successfully changing the way care is delivered.
