Community Health Center of Northeast Oklahoma, Inc., "The Road to Better Health", is a primary care health center providing quality primary care for individuals and families in Northeast Oklahoma. Our staff is caring, professional, dedicated and experienced. Our health services include; Women's Health Exams, Well Child Exams, Immunizations, Adult Physicals, Geriatric Medicine, Behavioral Health and Initial Medicare Exams. Our Disease Management to Include; Diabetes, Hypertension, Heart Disease, Respiratory Care and COPD/CHF. We have offices in Afton, Grove, Vinita and Welch to better serve our area. Hours are 8-5 Monday through Friday. A sliding payment scale is available for patients based on family size and income.
