Individual & Family Services - Boulder, Colorado, United States
Mission Statement: To actively support the inclusion of individuals with disabilities within our community.Community Link was established in 1958 by Boulder County family members who wanted to create expanded educational and employment opportunities for their relatives with disabilities. As a not-for-profit corporation, Community Link is governed by a local community volunteer board of directors. All monies received by Community Link are used for services to our customers and not going to structural overhead or shareholders. On June 29, 2016 Community Link's Board of Directors approved a plan to improve the quality of Community Link's Day Services. The plan describes the efforts to reduce the size of groups going into the community from five to three or fewer people. The plan utilizes a holistic approach to achieving the goals of program participants. It includes a renewed focus on employment for the people we serve. Implementation involves a seamless approach to the parts of a person's life (employment, day activity, and home life) through coordination and support of all areas by Community Link. The Service Excellence Plan requires the melding of a person's employment and day service funding with residential funding to improve service quality, provide service stability, and be consistent with new federal requirements around participant choice, service provision in the most integrated setting possible, and increased access to employment. Our Residential Department currently serves about 20 individuals in a variety of settings from Host Homes, and Family Caregivers to staffed independent living apartments. We are committed to expanding independence with each individual we serve, and finding the fit for each person regardless of the challenges.
