Crescent Health Institute (CHI) was founded in 1972 as one of the country's first community-based organizations to address the needs of people living with sickle cell disease and related hemoglobin disorders. The organization is headquartered in Charlotte, North Carolina with 501 (c)(3) tax exempt status. Our mission is to assure that those we serve can live healthy and productive lives through improvements in health, health care, and the services and support they need for well-being.Crescent services currently include testing, screening, counseling, education, case management, care coordination and support services.
