This innovative non-profit community health clinic provides health services to all, regardless of ability to pay. Uninsured are charged based on a sliding scale according to income level. The clinic provides primary care services including mental health services to all. Funding for the clinic comes from generous donors and grants as well as revenue from caring for individuals with insurance, Medicare and/or Medicaid. The clinic's model of care is based on the Six Dimensions of Wellness: Physical, emotional, occupational, intellectual, spiritual, social. All aspects of wellness are integrated. We have allied health professionals in the team to help address those needs: licensed dietitian, licensed counselor, and diabetic educators.The clinic also has a "Primary Care Employee Health Program" for employers to help reduce their health insurance costs while improving the health of their employees. This program provides integrated and coordinated care to employees. The clinic works with employers, the insurer and employees to develop strategies that will help reduce health care costs. An interdisciplinary health care team can be located at a clinic at the site of employment to make access to health care easier. The team works with each employee individually to help meet his/her health care needs in the most cost-effective way.
