Health Conscious is a digital health education and improvement platform that helps people with chronic health conditions, such as diabetes and high blood pressure, fully understand how they can remove the lifestyle risk factors associated with their conditions and take practical steps towards a healthier mind and body. The platform, available on both website and mobile app, consists of four online courses -High Blood Pressure, Prediabetes (CDC Prevent T2 Curriculum), Diabetes, and General Health. Each course consists of a weekly education video, group coaching call, action steps, community Q&A forum, and other interactive features. These include a Facebook group that hosts discussions, quizzes, and weekly group Zoom calls with the aim of creating a patient- centered experience. These lifestyle recommendations are consolidated by the American Heart Association, American College of Cardiology, and American Diabetes Association into an easy-to-follow roadmap. We also incorporate CBT techniques and stress management techniques as a key focus for health improvement, in addition to the traditional emphasis on diet and exercise, emphasizing a mind-body awareness.Health Conscious is supported by medical authorities from specialties ranging from Internal Medicine, Cardiology, Neurology, Nephrology, and Pulmonology, as well as preventive health researchers, psychologists, dieticians, and wellness coaches, incorporating the results from thousands of clinical studies. These various perspectives ensured that we help users gain medical understanding and then the action steps to implement that knowledge.
