The Falkirk Health and Social Care Partnership is one of 31 integration bodies in Scotland, serving a local population of 160,000 across Falkirk's rural and urban communities. The Partnership aims to enable people in Falkirk to live full and positive lives within supportive and inclusive communities. We do this through the integration of health and social care services. The Partnership's Integration Joint Board is responsible for planning, resourcing and operational oversight of services. The board is made up of representatives from Falkirk Council, NHS Forth Valley, the third sector, service users, and carers.Our workforce is employed by Falkirk Council, NHS Forth Valley, and our partners in the third sector. They work in health centres, GP Practices, community hospitals, care homes, intermediate care facilities and communities across the Falkirk area.There may be over 22,000 carers aged 16+ providing unpaid care in Falkirk. Unpaid carers provide care and support to family members, friends and neighbours. The people they care for may be affected by disability, physical or mental ill-health, frailty or substance misuse.
