The conversation about health care is a global human rights issue that warrants continuous discussion. The lack of access to quality healthcare has contributed to poor public health outcomes and higher mortality rates amongst our most vulnerable populations. To build community resilience, health equity must be embraced and the discussion on how to tackle health deserts and other impediments to care must be central. The Follow Up App was designed to address health disparities that are preventing ordinary people from accessing basic healthcare. Follow Up connects health care providers and close to five billion smart phone users worldwide, many of whom have health issues or are at-risk for poor health outcomes. Follow Up is a free app that provides a continuity of care after discharge from hospitals or urgent care centers. It increases the communication among the health care provider and the patients between routine outpatient visits. It is projected that this app will lower the rates of hospital readmissions which may occur as a result of post-discharge complications such as; (infections, fever, bleeding, etc.). Additionally, this app is expected to lower the overall costs for hospitals, third-party payors, and patients. The app is efficient and is designed to cut down on the amount of time patients spend in waiting rooms as well as increasing the patient flow during outpatient visits.
