Forward Care Medical Clinic (FCM) is one of the first clinics of its kind in the Lower Mainland. We operate on a shared care model between specialists and general practitioners in order to help provide comprehensive care to an aging population.At FCM, we believe that early team-based interventions will become necessary to help meet the health demands of what some have called the "aging tsunami". The current health care model, where a patient has to attend multiple different doctor's offices for their regular health care needs, is not ideal. For many patients, especially the elderly, travel arrangements can often be difficult and delays in the communication of health care plans can create complications in patient care, particularly when multiple specialists and/or language barriers are involved. To effectively address these issues and concerns, we at FCM aim to provide a "medical home" where all your care plans are kept in one centralized electronic medical record.Forward Care Medical Clinic is a teaching clinic. Doctors here are UBC Clinical Associates, which means that you may encounter medical students and resident doctors during your visit.
