Global Transitional Care is the first Group Specialty Provider organization of its kind. We employ an innovative clinically driven Transitional Care Model with the goal of keeping the patient from having to return to the hospital due to their primary or underlying co-morbidities. Through human resources supported by proprietary technology, our overarching goal is to prevent readmissions by elevating patient care, increasing efficiencies in post-acute care through real time, in-home clinical intervention.We provide a 30-day comprehensive clinical oversight, care coordination, patient/ caregiver advocacy and education, through a clinical transitional care team at no cost to the medicare patient*, healthcare facility, or additional provider *Medicare patients are subject to their standard Part B co-pay
