Through their work at previous care providers and then together as a team working at Grace, they have established a successful track record of building and ultimately selling several State and Medicare licensed facilities based in Michigan and in Arizona. The leaders of Grace Health Consultants decided to modify their business model as a result of changing demographics and the growing need in the health care industry to "bend the cost curve" downward. The leaders of Grace recognize that skilled medical services and mental health services delivered in a patient's home can be a significant contributor to the reduction of health care costs while enhancing the quality of life for patients and enhancing the effectiveness of treatment plans prescribed by physicians resulting in substantially fewer readmissions.By integrating tele-health and EMR based IT solutions connecting remote health monitoring and frequent visits with healing & prevention elements; patients heal and adjust to a new lifestyle faster, realize lower out of pocket expenses and experience reduced re-admission rates to hospitals.
