HC@Home enables patients with chronic diseases to actively participate in their treatment using a easy-to-use solution for use in the home situation. Working on their health in their own environment, with remote supervision by healthcare professionals, gives the patient full flexibility to select activities that fit the personal circumstances and preferences.With HC@Home activity and vital signs such as blood pressure, heart rate and weight can be measured. The data are automatically transmitted to a secure website via a specially adapted smartphone. De gegevens hiervan worden via een smartphone automatisch naar een beveiligde website gestuurd. Op ieder gewenst moment kunnen de metingen worden ingezien door zowel de patiënt als diens arts.
