HealthCraft designs, deploys and manages the delivery of patient support services through a network of pharmacies and other primary healthcare service providers.Patient support services are increasingly being employed to provide a ‘beyond the pill' value add to improve patient engagement, improve continuity of care as well as overall patient treatment experience. HealthCraft has available a growing menu of evidence based services for deployment and is able to undertake customised service design to achieve specific outcomes. Our well-grounded service design methodology enables us to deliver services more efficiently and mitigates service failure. Robust data management and outcomes measurement is achieved via a secure web-based platform. All services are delivered through a Managed Services Network which is specifically trained, audited and accredited to provide the required service capability. HealthCraft supplies implementation support and on-going performance management of the network in order to maintain service quality standards and to provide for continuous improvement opportunities. Service evaluation is an integral part of patient follow up.
