The Healthy Community Alliance is a rural health network chartered in 1996 by key community leaders concerned about maintaining and improving local health care services during a time of great change. Our service area encompasses portions of four Western New York counties along Route 39 – Cattaraugus, Chautauqua, Erie, and Wyoming. Within that area are some 3,100 Native Americans residing on the Cattaraugus Indian Reservation; 200 Old Order Amish; 16 school districts; and four NYS prisons housing more than 7,000 inmates.Some of our programming includes:• Navigator Program - HCA is a designated Navigator agency for the NY State of Health, a healthcare marketplace. They help get qualified individuals free or low cost health insurance.• Patient Activation Measure (PAM) - A program designed to target Medicaid populations and help them take advantage of preventative medical care, where they may not have been doing so.• Partner with Springville Concord Elder network (SCENe) to establish a "one-stop" for senior services• Falls Prevention - Education, programs, and activities throughout Cattaraugus County.• Safe Routes to School Initiative- Focuses on the importance of providing safe routes for children to walk and bike to school.
