Heart Wellness Clinic Inc. dba House Call Medical Professionals serves as both an in house clinic, and a mobile clinic, providing quality internal and family medical care to patients in the Southern California area. We are committed to educating the public on the primary and tertiary prevention of their overall cardiac health, using an individualized and compassionate approach appropriate for each individual patient. We hope to teach the community on how to prevent cardiac episodes and practice a healthy lifestyle, while preventing hospital readmission within 30 days of discharge. Areas where we can serve your needs are: COMPLETE HISTORY AND PHYSICAL EDUCATIONAL LITERATURE1 on 1 EDUCATION ON DISEASE PROCESS BY OUR MEDICAL PRACTITIONERS SUCH AS MDs, FNPs or PAs. DIABETIC CLASSES WITH EMPHASIS ON DIET AND EXERCISE. SMOKE CESSATION HEART FAILURE REHABILITATION STRESS REDUCTION PROGRAMS SUCH AS…. YOGA CLASSES/EXERCISES PSYCHOLOGICAL EVALUATIONS AND TREATMENT FOR ANXIOUS AND DEPRESSED INDIVIDUALS.Unfortunately, because of the number of responsibilities of our healthcare professionals in the clinics and hospitals, patients are often-times deprived of adequate teaching about their disease process and preventive measures against re-occurrence. We offer in clinic and house call care, either acting as the patient's PCP or collaborating with the patient's PCP (if they already have one) to provide quality care and proper health education.Our Patient population includes but not limited to; Congestive Heart Failure (particularly for prevention of re-admissions into the hospital within 30 days).
