The Hendrich II Fall Risk Model™ (HIIFRM) is one of the most widely used fall risk assessment tools at more than 1,000 sites of care throughout the U.S. and abroad. The tool predicts injurious falls while promoting the larger opportunity for safe mobility and healthy aging. With just eight assessment parameters, Rate 8©, the Model uses five intrinsic risk factors, two classes of medication and a gait and mobility test to establish a person's baseline for care plan interventions. It appears in more than 300 peer-reviewed articles and 30 validation studies. The HIIFRM is appropriate for use across the care continuum, population health entities, and clinic environments to improve quality, safety and the cost of care.
