Home Recovery Rx provides practical support to older adults returning home from the hospital. The focus is on ensuring the home environment provides them the control and comfort to heal efficiently and return to their normal lives. Patients with more personal control have more independence, place less burden on caregivers and heal more effectively than those who struggle with simple activities we take for granted when healthy (e.g., turning on the lights, climbing stairs, using the bathroom, preparing meals. The greatest limiting factors experienced by patients and their loved ones are non-adapted environments, confusing equipment requirements and choices, "dumb" homes and un-leveraged support. The Home Recovery Rx solution replaces these with Home Fit temporary home adjustments, Home Equip equipment paks, Home Smart technologies, and activation of Home Team support networks.
