Human Telemetry has developed connectivity between one of the world's largest Heart Failure Defibrillator manufacturers and the Salesforce1 platform. By means of this new connectivity, daily individual heart failure status data is routed from the heart failure patient's implanted defibrillator to the patient and their physician via a Salesforce1 based mobile application named BioThrive Cardio. Biometric heart failure data within patient's defibrillators are proven in clinical trials to markedly reduce heart failure readmissions. Yet for most physicians, the actionable heart failure data is received days to weeks too late, such that the data has lost the potential to help avoid heart failure exacerbations and readmission. For the first time ever, patients can independently receive this important data. Also, physicians can receive this now actionable data on a daily basis and apart from the process delays of Electronic Health Record workflows. This paradigm change in device data management is of critical importance as congestive heart failure is the most expensive disease in the United States, in fact, more expensive than all cancers combined. Human Telemetry puts the patient upstream of their Heart Failure Status rather than downstream and thus at risk of an avoidable adverse clinical event. Human Telemetry is different because in addition to exposing critical information near real-time, we are also creating a full CRM solution designed around the patient's needs and experiences rather than designed for legacy institutional protocols and workflows. Our solution allows for what we feel is the critical trifecta of engagement between: (1) the patient with their own physiological data for disease management, (2) the patient's physician who is now receiving time-sensitive and actionable data, and (3) the manufacturer of the patient's device for continuing education and product performance feedback initiatives.
