The personal impact that pain conditions can have on patients has led to an over-reliance on opioids and the alarming epidemic of opioid overdose deaths and abuse. The National Academy of Medicine states that health professionals' primary role for chronic pain should be guiding, coaching, and assisting patients with day-to-day self-care. However, this is rarely done as healthcare providers lack the time, training, tools, and reimbursement to guide patients in self-care. Furthermore, when the pain becomes chronic, it is associated with depression, missed work, disability, and intensive use of high-cost high-risk interventions such as opioid analgesics, multiple medications, interventions, and surgery. Half of people seeking care for pain conditions continue to have pain five years later despite treatment due to patient environmental risk factors that prevent self-care.We developed an innovative digital platform called Personalized Activated Care and Training (PACT) to provide health professionals and their patients with a reimbursable personalized patient-centered whole person self-management training program to implement as part of routine care for pain conditions. PACT technology is an online platform, like an electronic health record, that includes telehealth coaching, pain risk assessment, self-management training, remote monitoring, pain resources, billing, data analytics and more to help patients prevent chronic pain, addiction, and their consequences.Our 6-month therapeutic program guides patients in patient-centered strategies to relieve pain conditions and prevent chronic pain, delayed recovery, substance misuse and the opioid crisis.
