This interoperability brings an end to information silos with medical data. The data is still stored in the silos, only the reference and the authorisation are stored on the blockchain. This is done based on the information standard Registratie aan de Bron.WHATAn app enables the healthcare professional to request access to the data of a patient. The patient can grant access after he received the request on his own device. Then the authorisation for this professional is stored on the blockchain. The patient has already indicated which health organisations are filing his medical data from the past.When the professional opens the Lemon app, he will see one timeline with all combined information of the health organisations where his multi-institutional medical data are stored now.HOSPITAL @ HOMEThe h@h case reflects very much the wider case of a patient sharing data with several professionals.
