Our agency is here to advocate, identify and coordinate services that will provide support for disabled individuals who have met eligibility requirements.Free Services that may be available to you include:Community Transition Services (available only through Medicaid Home and Community Based Waiver Services), Participant-Directed Community Supports, Personal Emergency Response System (PERS), Service Coordination, Home Accessibility Adaptions, Equipment, Technology & Medical Supplies, Home Delivered Meals, Personal Assistance Service, Service Coordination.WAIVERS SUPPORT SERVICES ****Attendant Care*********If you have a physical disability, the Attendant Care Waiver and state funded Act 150 program may be available to you to continue to live in your home an community with support and services. To be eligible you must: Be a resident of Pennsylvania, age 18-59, Meet all state requirements ****Independence Waiver******** To be eligible for the Independence Waiver you must: Be a Pennsylvania Resident, ages 18-60 Individuals that turn 60 while in the waiver will be able to continue to receive services through the Independence Waiver. Meet ALL and financial requirements as determined by local county assistance office.****Aging Waiver ****** Services to be eligible: Be 60 years of age of Older, Resident of Pennsylvania, US Citizen or qualified Non-Citizen Meet the level of care needs for a skilled Nursing Meet financial requirements as determined by the local****Act 150****For individuals age 18-59 who have been assessed to require services at the level of nursing facility level of care. This program may assess a minimum copayment for services.
