Based in Cook County, MyCare coordinates patients and their doctors to control costs and ensure a healthy population.MyCare was created by doctors, clinics and hospitals that have been treating the Medicaid population for many years, and provides its patients with 24 hour, 7 day a week access to a member of their healthcare team in order to ensure they get the best care available.MyCare Chicago is made up of:Four hospitals (Ann & Robert H.
