We are a clinical pharmacist service provider working closely with GPs in South Australia. Our pharmacists provide tailored clinical services to suit the needs of the practice. These include; medicine reviews, medicines information, chronic disease management, health promotions, education and general advice on diet and lifestyle. We are essentially a support service for GPs, working closely with them and their patients to find the best solutions for their overall health and well-being.We believe better collaboration between health professionals and greater follow up of patients can greatly improve the continuity of care, leading to better outcomes.With an ageing population and and a rise in chronic diseases, primary health care needs to evolve in order to cope with the increasing pressure. Patients need greater access to services which can assist them in managing their conditions. Our model of care focuses on just this. We support GPs in their role to keep patients healthy, happy and living actively in the community. We are proactive and strongly believe that prevention is better than a cure. Patient education and empowerment is key to a successful treatment plan and this is one of our main objectives. We also offer diabetes educators and health coaching services.
