National Care Management, is a group of committed healthcare professionals and innovators helping Australians with conditions associated with ageing and other complex health needs or disability to live healthy, independent, socially engaged lives at home. With decades of experience in aged care, healthcare services and community care, our expertise is in navigating the health, community and aged care systems, with the goal of helping clients find the right support services and, wherever possible, access them in the comfort of their home. Care Management is a new, evidence-base discipline in the Australian health system. It takes the traditional role of case manager, the findings of best practice research, and a holistic view of each client's situation. This is combined with low case-load management, to ensure there is focused commitment to each client. As part of its commitment to ensuring lowest cost access to high quality care, National Care Management works with select private health funds to provide access to Care Management services free-of-charge for eligible customers. The care management team also works with all sources of government and community funding to ensure clients receive the very best service whilst managing a budget.
