Nightingale NJ is dedicated to assisting older adults navigate the healthcare system and maintain control of their lives throughout the aging process. An Eldercare Navigator acts as a liaison between the client, their family members and healthcare providers, with the goal of improving continuity of care. We help seniors and their adult children to make sound choices when confronted by the confusing array of medical, financial, legal and emotional matters related to aging.Our goal is to ensure that older adults live at their desired level of independence. Our Eldercare Navigators help keep life simple by guiding you in making proactive, purposeful decisions aimed toward that goal. We meet with you to find out what's important to you, and complete a series of need-based assessments. We then create a Care Plan with short- and long-term strategies that affirm your values, maximize your strengths, and facilitate your independence.We visit regularly to ensure the plan is being followed and things are on track. Our Eldercare Navigators are experienced Nurses and Social Workers adept at recognizing changes in the elderly and taking action to prevent small concerns from becoming big problems. We keep family members and professionals informed as to the well-being and changing needs of the client so everyone's on the same page, and we help family members find common ground when they are in disagreement with the client or one another.NightingaleNJ Eldercare Navigators address universal questions with local knowledge. We have a network of trusted providers cultivated through our professional roles as well as our personal lives. We accompany to doctor appointments, find appropriate housing, connect with community resources, provide crisis intervention and refer to financial and legal experts as needed. When the right direction is unclear, we advise with clinical expertise, professional experience, and one bias only: the client's best interest.
