NP Housecalls collaborates proactively with your surgeon, specialist and/or primary care provider, your family and/or residence facility staff to ensure that you receive the collaborative care and resources requisite for your optimal health outcome.Palliative CarePalliative care is a multidisciplinary approach to specialized medical and nursing care for people living with chronic conditions and/or a life-limiting illness.
