Obesity PPM was an innovative disease management company improving population health via patient engagement and care coordination interventions addressing obesity, type II diabetes, and myriad related conditions. We served healthcare delivery organizations transitioning to value-based reimbursement, medical device and pharmaceutical companies driving commercialization and utilization of evidence-based treatments, as well as health plans, managed care organizations, Accountable Care Organizations (ACOs), and other healthcare payers seeking to make significant health economic impacts in the populations they serve. Delivered under the brands of its customers, they fully integrated three service areas to design and administer novel disease management programs: engagement center operations with experienced navigators, health coaches, and educators; HIPAA-compliant cloud-based health IT systems with sophisticated mobile applications; and a broadly multi-disciplinary consultancy comprised of thought leaders in all aspects of obesity and its comorbidities. Our leadership team brought more than 280 years of combined obesity treatment experience to program design, administration, monitoring and evaluation. Our patient, provider, payer and population engagement services transcended organizational boundaries and orchestrated disparate processes. We activated populations and successfully navigated patients through complex, individualized treatment journeys defined by clinicians' unique care plans, while delivering powerful analytics that advanced patient experience, transparency, quality, and cost efficiency.
