Our mission is to promote health equity, actively participate in the revitalization of historically underserved communities, and build leadership capacity through service, education, and research.We have a long history of developing partnerships with community-based organizations that have a similar mission. Our scope of work includes programs that provide direct services and others that increase community capacity to develop, sustain and monitor programs to meet the needs of specific populations.Since its inception in 1993, OCEAN-HP has received more than 35 grants totaling over $13.8 million from federal, state and foundation sources for its innovative and comprehensive approach to neighborhood development, institutional commitments, and university-community-government partnerships. OCEAN-HP has also received local and national recognition for its achievement.Who is Eligible?OCEAN-HP services the community at all stages of life and the primary focus is towards the historically underserved population due to systematic racism. We also provide resources and information to community partners and other entities or departments within the University of Illinois system.Our major areas of concentration include:• Health professional and lay health worker education• Consumer health and wellness education• Community-engaged research, research dissemination and implementation• Community-based health services including primary and behavioral healthcare services
