We are a group of independent professionals serving seniors, individuals with disabilities and their families in Northern Illinois. Our advocates are health and human service specialist who help our clients optimize their quality of life by shining light on care options.Services• Our Advocates conduct Comprehensive Face to Face Assessments addressing the medical, emotional, functional, safety, financial and legal needs of our clients and their families whether they reside at home, in an assisted living, nursing facility or a residential facility.• Advocates develop an Individualized Plan of Care based on the clients' desired quality of life, assessment findings, recommendations, care options, service and support resources. • Our Advocates guide the plan implementation process between client, family, doctor and other professionals streamlining communication and building networks of supports. Our Advocates can also fulfill the needs of Trust Officers, Attorneys and long distance caregivers.• Ongoing Care Management monitors the needs of clients, coordinates care, and monitors the quality of the supports and services. Advocates can provide health education for clients and/or their caregivers, home safety monitoring, health care compliance supervision and medication oversight. Advocates continually monitor the plan and make recommendations for adjusting the services as indicated by changes in the level of care.
