Partners for a Healthy Community is a 501(c)(3) not-for-profit organization established in February 2021 in response to the COVID-19 crisis. Founded on the social theory of collective impact (CI), Partners for a Healthy Community leverages community resources and establishes cross-sectoral partnerships to overcome challenges and barriers to holistic health and well-being.Our mission is to close the digital gap and improve technological literacy and health outcomes among vulnerable seniors. The acceleration of healthcare reliance on technology worsened an already alarming digital gap among seniors, deepening their isolation and exacerbating many prevalent mental and medical comorbidities among this vulnerable population. By providing access to technology and the necessary education to build the skills to utilize it, our community-based organization aims to improve mental and medical health among seniors and ultimately change the landscape of healthcare delivery and its outcomes.
