She reviews all medical issues to assure that appropriate medical attention is being carried out, and then develops a Plan of Care which addresses the client's individualized needs along with a specific plan of how best to meet them. A plan of care may include: coordination of home care, installation of medical equipment, referrals to physicians or elder law attorneys, attendance at key medical appointments, updates to family and continued contact to ensure that any changes in the client's status are addressed on an ongoing basis.
