About Pediatric Partners of the SouthwestPediatric Partners of the Southwest was founded in July 2005 with the mission of providing high-quality, compassionate, and holistic pediatric care to the babies, children and adolescents of our community. We have compiled a wonderful team of board certified Pediatricians, Physician's Assistants, and Pediatric Nurse Practitioners who specialize in the care of infants, children and young adults. Pediatric Partners' board-certified pediatricians also serve as Mercy Regional Medical Center's only inpatient pediatricians, providing around-the-clock coverage for newborns, critically ill and premature babies, as well as hospitalized pediatric patients. Our goal is to partner with parents and guardians to provide every child excellent, comprehensive, and informed healthcare.PPSW has implemented a comprehensive, team-based pediatric medical home system in which the Pediatrician or Advanced-Practice Provider uses the expertise of other professionals in order to offer whole-person care to ensure all aspects of a patient's well-being are addressed. Pediatric Partners, with the assistance of Rocky Mountain Health Plans, has started a Behavioral Health program within our office. We have Behavioral Health providers who are present in the office Monday-Friday. They will be able to provide targeted intervention for common behavioral and developmental challenges such as potty training, tantrums, or bedtime; they can help with management of chronic medical conditions, including adherence to treatment; assist with parental transitions (post-partum depression, returning to work, divorce); and they will serve in a critical manner to help with the interface of school and community mental health programs with the medical home.Pediatric Partners of the Southwest is committed to providing excellent Pediatric primary care to our community. This now includes pediatric care in school settings! Silverton Schools has a Tele-Health linkage with PPSW, whereby a child and parent visit over an internet video linkage with the physician, and are examined by the Silverton nurse using state-of-the-art otoscopes and stethoscopes that transmit live, accurate video and audio data to the physician. When the passes are closed, Silverton kids can still be assessed, prescriptions can be written, and health issues can be solved, often avoiding a long drive into town. Mountain Middle School, a Durango charter school, has a student-based health center as well. A PPSW physician will be present at MMS 5 half-days per week, and a nurse will be present throughout each school day. If illness or injury arises when the physician is not on campus, a Tele-Health visit can still happen with one of the great providers at Pediatric Partners of the Southwest. A Behavioral Health Clinician will also be ready to assist the physician/nurse, modeled similarly to the highly successful integrated Behavioral Health program in the PPSW main office.
