The Premier Health Network is an Arizona based health care agency designed to service medical practices and health care providers in the areas of program development, implementation, and management of enhanced care services.PHN serves primary care and specialty providers (physicians , specialty medical groups, and NPP led practices) valley wide in an effort to grow their practices through the implementation and management of enhanced patient services and specialty programs, such as Chronic Care Management, Principal Care Management, and Oncology Care Model, in order to drive additional revenue while enhancing the patient experience, leading to greater treatment outcomes and increased quality.PHN employs social workers, Nurse Practitioners, and certified Navigators to provide advanced care planning services to patients within the practice to monitor and manage chronic conditions in order to reduce acute episodes and hospital admissions with the goal of driving down the aggregate cost of such medical services to health plans a d Medicare.PHN aims to serve the individual providers who wish to participate in CCM and OCM as well as specialty and ACO affiliated physicians groups who aim to reduce cost while participating in these same CMS initiatives.
