Team Around the ClientAt Rebuild 4 Life we build a team around the Client. We co-ordinate with a multi-disciplinary team of professionals who follow the Client through their rehabilitation journey, maximising their potential, and supporting them to manage the long-term effects of their injury to maintain and improve the client's quality of life, function, independence, social and vocational opportunities.The role of the Rebuild 4 Life case manager includes assessing the client's needs and supporting clients to recruit and manage a bespoke support package, procure services from various care and therapy providers. We monitor the service provision ensuring quality and cost effectiveness. This is a collaborative process, which involves establishing a therapeutic working relationship with the client and significant others.At Rebuild 4 Life all our Case Managers are Registered Health Professionals and Advanced Registered Practitioner with The British Association of Brain injury and Complex Case Management (BABICM).
