Reliable Home Health Corporation opened its doors on May 25th 2004. We started out as a specialty infusion company working with local compounding pharmacies administering their medications. By 2006, Reliable Home Health Corporation achieved Medicare accreditation and also began offering physical therapy for our client's patients. During the next few years we steadily grew; adding 8 more pharmacies to our client portfolio until 2009. That is the year we also earned our accreditation from ACHC.Seeing the need for home based chemo therapy, Reliable Home Health staff became certified to administer these life extending medications. We worked extensively with Sylvester Cancer Center where we excelled. Since 2012, Reliable Home Health has responded to the need of both patients and health plans to better manage those high risk patients that keep readmitting. These members with chronic conditions were falling through the cracks and there were few options available to the health plans. Reliable Home Health's Chronic Care and Care Transitions programs addresses both the immediate needs of post discharge patients through our Care Transitions and the long term management of those patients through our Chronic Care programs. Through our efforts, we have dramatically cut 30 and 60 day re-admissions while reducing year over hospital admissions for those chronic patients.
