Our challenge, as orthodontists, is partly about "seeing" our patient's uniqueness - in what they want from our therapy, and partly about our recognizing their anatomic boundaries inside of cortical bone. As with most things biologic, there is a natural phenotypic distribution in our patients' alveolar foundation; some anatomic challenges are beyond difficult. Using CBT imagery, it becomes more clear that treatment mechanics for one Class II case type may well be injurious for another. The challenge is very evident in our patients with a long lower face pattern; with data showing significant risk for iatrogenic periodontal damage in this orthodontic case type. I believe the alveolus is actually what will drive our specialty going forward, more than simply "aligning teeth" and "fixing bites." If orthodontics is not periodontally-focused, its a bit analogous to not seeing the consequences of hydrocarbon production on air quality; there is more than a simple system at play.
