Scrypt Health is the tech-enabled chronic disease management company that offers customized treatment programs that empower patients, allowing them to embark on their journey to achieving optimal health. Our patient centered programs offer an effective alternative to the traditional health care delivery model in the United States, which we believe underserves chronic disease patients. While our system is well set up for episodic care, we believe it does a poor job of managing chronic illness over long time periods. This structure of episodic encounters results in lower compliance rates for a chronic patient's plan of care, resulting in subpar outcomes for segment of the patient population. Scrypt Health's unique approach focuses on patients' overall wellbeing, and offers a platform that provides safe, effective, and convenient care for chronic illness. Our high touch service features personalized attention from an empathetic concierge, resulting in a sustainable long term provider-patient relationship. By monitoring a patient's condition over a long time period, we're able to effectively shepherd them through their journey to optimal health. This journey consists of an initial assessment (including diagnostic testing), followed by scheduled adjustments to stabilize the patient's condition and monitor them at regular intervals to maintain this achieved stabilization. By capturing, measuring, and presenting empirical data to our patients via our "patient scorecards," we're able to provide real life evidence of the improved outcomes we provide to our patients.
