The process begins with an assessment that examines the individual's financial, emotional, physical, and cognitive necessities. Based off the assessment's results, licensed nurses determine if the individual needs an assisted living community, home care specialist, or neither. If the individual requires home care or an assisted living community, the nurses take steps toward connecting the individual with the desired long-term care service.All services are free of charge to the client. This ensures the client receives the best possible care, not the most expensive.
