SET's vision is a vibrant, productive Milwaukee where people develop to their full potential.Achievements•In 2012, SET's Adult Services-Family Care Team was ranked 1st among all Care Management Units in Milwaukee County.•SET received the first-ever President's Community Impact Award in 2011 from the Medical College of Wisconsin in recognition of the work SET does to improve the health of the community.•SET received a Best Practice Award from the U.S. Department of Housing and Urban Development and the U.S. Department of Health and Human Services for services-enriched public housing. In 2003, SET's unique model was published as a case study of a high-performing partnership by the National Academy of Public Administration.4,500 people, including 2,500 seniors and adults with disabilities, benefit from SET's evidence-based methods each year from 22 locations in Milwaukee:•Grandparents: 2,500 senior and disabled adults maintain their health and age in their own homes exercising their own decision-making in Milwaukee's 14 public housing facilities. •Parents: 1,000 families and individuals are guided each year to establish and fulfill goals to increase resiliency in children and promote family stability.•Children: 1,000 elementary school students develop life skills in emotional regulation, problem solving and decision making.
