Silverton Community Care (formerly Palliative Care for Utah) provides in home Primary Care to our patients. We offer services such as:❖ MD Visits❖ NP Visits❖ In-Home Phlebotomy❖ In-Home Imaging (X-Rays)❖ Coordination of Specialty Doctors/Clinics❖ Coordination of Home Health Care❖ Coordination of Hospice❖ Coordination of Pharmacy ServicesOur Medical Director is Dr. Mark Martinez MD, MBA. He is board certified in internal medicine, pulmonary and critical care and has over 15 years of experience managing patients in the post-acute care setting. Dr. Martinez has been responsible for overseeing the Palliative Care for Utah patients and will continue to care for them with Silverton Community Care.We want to provide the best care to all of current patients and new patients.
