The Brooklyn Health Home (BHH) was established to coordinate medical and behavioral healthcare services for Medicaid patients. It is built on the foundation of a successful program developed at Maimonides, along with many community-based organizations with funding provided by New York State.The BHH currently partners with care management agencies including hospitals, community-based organizations, and social service providers across Brooklyn to identify and address the full range of needs affecting chronically ill patients. The BHH connects qualifying individuals with physical and behavioral health providers, as well as their own own care manager to support and advocate for their needs.
