WCHAP provides health education to families, helps doctors improve the quality of their care, and acts as a voice for issues affecting children's health. \\WCHAP has a multi-disciplinary team who work with doctors who refer families for a variety of reasons including helping families understand how to access a ride to their child's doctors' appointments, to staying on top of their child's shots, to helping manage asthma, and to offering a free healthy lifestyle program---Fit Kids 360—for the whole family. Both parents and doctors want children to be healthy so they can be healthy adults. But there are many barriers that can get in the way of doctors and parents working together. WCHAP works as a link between families, their doctors, and their Medicaid Health Plans to make sure children have the best possible health and healthcare.
