Our practitioners are trained in the field of Palliative Care and have extensive experience in structuring these difficult conversations as well as securing personal preferences of care. We utilize a secure, video guided tool that helps you understand your treatment options and align personal goals of care with available treatments. During an online visit, our practitioners will assist you or your loved one to complete the form, share it with appropriate physicians and family members, and answer questions about what is most important to you. We will work to clarify your understanding of what Palliative Care and Hospice care are.
