Queensland's leading chronic disease prevention and behaviour change program.My health for life is a free, six month behaviour change program designed to assist Queenslanders reduce their risk of developing chronic diseases such as type 2 diabetes, heart disease and stroke. Funded by the Queensland Government; through Health and Wellbeing Queensland; and run by the Healthier Queensland Alliance (Diabetes Queensland, Stroke Foundation, Heart Foundation, Ethnic Communities Council of Queensland, Queensland Aboriginal and Islander Health Council and all Queensland Primary Health Networks), My health for life has helped over 10,000 Queenslanders get their health back on track by making small, sustainable changes.Delivered by local health professionals across the state, the program can be delivered via: group sessions within community (face-to-face or videoconferencing), one-on-one telephone health coaching or as a workplace program.
